I have not seen destruction of an AR upper receiver like this before. The left side of the upper receiver has been peeled open almost like a banana peel. Marky of Tactical Tshirts condemns the cartridge because it is too easy to mix up ammo and have a catastrophic failure.

I'm seriously worried now, I have a load of 300aac ammunition in my ammo safe, what it I put it in my 22lr?, or my 308 or 6.5's? how can I prevent such an obviously easy mistake? it's all too difficult

Happens with shotguns on a regular basis ... drop a 20bore into a 12bore and it slips down. Put a 12bore on top and you have a problem!
I have seen it done, but not fired fortunately. One couple I know shoot a 12 and 20 ... the husband may have a few 20s in his pockets even though he is shooting a 12. Too much of a risk.
20bore cartridges were always Yellow and nothing else was, as I understood it, a safety precaution; recently I have seen 12bore in Yellow - why!

I did not know that being relatively new to 20's, interestingly all of the new 20bore cartridges I buy are yellow however I have a few old paper 20 bore in assorted colours

It was my understanding that because of the potential problems there was an "agreement" to make all 20bore cartridges in on colour - Yellow was chosen, and other sizes would not use that colour. Obviously there are still large amounts of old paper cartridges around which could be any colour.
It is only teh 20/12 mix that there is a problem with. I shoot with 12 and 16 - and dropping a 16 into a 12 barrel will stop a 12 going in so not a major problem- getting it out might be!
